Auxiliary

Agents used to reset a system that stopped regulating itself — gut motility, autonomic tone, appetite and safety signalling. This is the thinnest evidence base in the catalogue and every item is an individual conversation.

P

Prucalopride

Off-labelOral

5-HT4 agonist. Asked about as a gut-brain motility reset.

Evidence

Solid trial evidence for chronic idiopathic constipation. The motility-reset use — a defined course, then stopping, with fibre reintroduced — is not studied. Reports of it are anecdotal, including from people who found nothing else worked.

P

Pyridostigmine

Off-labelOral

Cholinesterase inhibitor. Asked about for autonomic dysregulation.

Evidence

Approved for myasthenia gravis. Small studies in orthostatic intolerance and POTS show benefit for some patients. Requires a cardiac history before starting.

Oi

Oxytocin, intranasal

Not offered
CompoundedIntranasal

Asked about for appetite, safety signalling, and social anxiety.

Evidence

Approved only as an injectable obstetric drug. Intranasal formulations are compounded and investigational — trials in social and anxiety contexts are small and inconsistent. Not currently offered.
